In order to complete the PTA Program Application you will need electronic copies of the following documents:
- Transcripts from all schools attended
- Substitution Course Form (if applicable)
- Courses in Progress Form (if applicable)
- HESI results sheet
- Essential Job Functions Form
- Observation Hours Forms
- Essay
